Report

What Constitutes a Report?

• A formal report comes through your intake office

• Report to central reporting line (phone or email)

• Someone appears in hospital

• The adult self-discloses

• Someone reports concerns to any staff

• Staff observes something of concern

Ask Yourself…

• Is there a reason to believe this adult may be unable to seek support and assistance?

• Is there suspicion of abuse/ neglect/ self-neglect?

If the answers to these two questions are “yes”, the report should be assigned to a DR.
