Cross Program Involvement

The adults’ journey through the health care system will take them through many different programs and services and will involve many different health care providers.

• Transitions are high risk times for adults who are unable to seek support and assistance as the DR who conducted the investigation will be transferring the details to another program.

• Ensure thorough communication with the DR or DRC who will be receiving the case when transferring to another program area.

• Determine who will take lead responsibility for investigation and follow-up.

• Provide full documentation to the new Designated Responder, especially if there is information in a paper file that can’t easily be accessed from the new location.

• Do not “hand off” an investigation until another Designated Responder has taken responsibility.
